Athletic Participation Paperwork Process

The Columbia High School Athletic Participation Packet is digital through Final Forms, the same platform that student registration is completed.  If your student is participating in any Wildcat Athletics, you will need to complete the required paperwork online. 

Use your Final Forms Account you used to Register your student-athlete for school.

Final Forms

If your student is a Freshman or Junior, they will need to get a sports physical prior to the start of their season.  This will be uploaded in Final Forms.  If you have questions, please call our school at 208-498-0571.
